How to Apply for DVA Educational Benefits

 

 

Students new to Wayne State

First Time Using DVA Benefits

1. Complete application for Veterans Educational Benefits.

DVA Form 22-1990 (Reservists/Veterans/National Guard) OR
DVA Form 22-5490 (Dependents)

2. Turn in completed application with:

DD214 (Veterans)
NOBE - Notice of Basic Eligibility (Reservists/National Guard)

NOTE: Dependents/Survivors need to know VA claim number prior to applying.

3. Complete Request for VA Enrollment Certification.

4. Complete OMVA Student Record Form.

5. Turn in a Plan of Work form signed by an Academic Advisor.

Previously Utilized DVA Benefits (Transfer Student)

1. Complete Request for Change of Program or Place of Training.

DVA Form 22-1995 (Reservists/Veterans/National Guard) OR
DVA Form 22-5495 (Dependents/Survivors)

2. Complete Request for VA Enrollment Certification.

3. Complete OMVA Student Record Form.

4. Turn in Plan of Work or Degree Audit signed by an Academic Advisor.


Students Returning to Wayne State and Continuing to Utilize Benefits

Complete Request for VA Enrollment Certification.
