Code of Responsibility Form

#1 Add/Override Approval Form
  This form is used if you receive an error when trying to register for a class and may need prior approval. After obtaining the instructor's signature, this form should be taken to the department to have the applicable override put in the system. If it is within the first 2 weeks of classes, the course can be added through Pipeline by the student. After the 2nd week, please follow the detailed instructions noted on the form.
   
#2 Address Change
  Students who have access to pipeline are able to update their own address.
   
#3 Application for Degree or Certificate
  This form is now online. The above link will take you to pipeline. Click the Student Tab > More Student Services link > Student Academic and Financial Records link > Apply for Degree or Certificate link.
   
#4 Application for Residence Classification
  A non-resident student may apply for resident status if s/he believes that circumstances warrant it. Resident status, if granted, will allow the student to enroll in future terms at the resident tuition rates.
   
#5 Enrollment Verification/Certification Request
  For loan deferment processing, enrolment verification and degree certification.
   
#6 FERPA Release
  This form is used to allow students to authorize other individuals such as parents, guardians, etc. access to their educational records relating to grades, course performance, disciplinary proceedings, tuition and fees, schedules, and financial aid. #7 Michigan Uniform Undergraduate Guest Application
  This application is for Undergraduate Students Only.
   
#8 Name and Address Change
  To initiate update of student record with new or corrected name or address data.
   
#9 Petition for Credit By Examination
   
#10 Release of Directory Information
  The University has changed its policy in regards to the release of students’ directory information.
   
#11 Request for an Exception to Enrollment Policy
  To request special exceptions to deadlines for registration, drop/add, or tuition and fee cancellation.
   
#12 Request for Medical Withdrawal
  To request complete withdrawal from classes for medical reasons, with full or partial tuition reimbursement depending on when the withdrawal occurs.
   
#13 Request for Waiver of Non-Resident Portion of Tuition
  This program is terminated effective Fall Term 2008 due to a new Residence Classification Policy. A,E,G,H,I,L,R and TN Visa holders and military service personnel may apply for resident classification under the new policy. Detailed information is available by reading Regulation 6 for Non-U.S. Citizens or Regulation 7 for military personnel which is included with the application.
   
#14 Returning Student Record Update
  In situations where a student has not registered in several years and finds s/he cannot register, this form initiates review of student’s record to identify and address barriers to registration.
   
#15 Transcript Request
  This form may be used to obtain official or unofficial transcripts.
   
#16 Withdraw from a Class
  Instructions on how to request to withdraw from a class via pipeline. It is the student’s responsibility to request the withdrawal. Failure to do so will result in a grade of F.
